ABOUT JACQUI
Jacqui is an Arts Based Psychotherapist, Counsellor, and Practicing Artist.
Jacqui’s experience spans over 18 years of working with children, adolescents and adults (particularly mothers) as she walks along side them to explore different ways of being in their lives.
Jacqui provides a non-blaming, multimodal approach to her Art Based Psychotherapy work. She uses arts-based processes to facilitate self-expression, communication, self-awareness and personal development. No artistic ability is necessary. It is through being together that we can make sense of our lived experience. As new understandings emerge we can find ways to develop, and live differently.
Alongside her Arts Therapy work, or exclusively if required, Jacqui also offers counselling sessions that may include aspects of Acceptance and Commitment Therapy, Narrative Therapy, Mindfulness and CBT as required.
Jacqui holds a Master Degree in Experiential and Creative Arts Therapy from the MIECAT Institute. She has a background in teaching including Rudolf Steiner training, and has undertaken training in Narrative Therapy, ASIST (Suicide Intervention), Mindfulness, Depression and Anxiety (including OCD and Perinatal specific) and Acceptance and Commitment Therapy.
Jacqui holds a professional membership of ANZACATA, A member of Australian Counselling Association (Level 2), is a registered teacher, and has a current police check and Working With Children Check.
CONFIDENTIALITY
Confidentiality will be maintained expect if there is concern for the safety of client/participant or others. Jacqui will sometimes discuss sessions with her external supervisor maintaining privacy of names.
NOTES/ARTWORK
Notes may be taken by Jacqui during a session, and clinical notes will be completed after a session. These notes will be kept in a locked file on computer in Jacqui’s office until such time they may be destroyed.
Access to notes may be made in writing to Jacqui.
Artwork not immediately taken by client will remain the property of Jacqui and filed as above (when appropriate). Jacqui may use photographs of these works for examples, future advertising or in written papers regardless of whether consent has been asked.
FEE POLICY
Individual sessions:
Service includes a face to face appointment of 50 minutes-1 hour.
24hr cancellation notice required.
Fee will be negotiated if cancellation is under this time depending on circumstances.
Fees need to be settled prior to, or on the day service is provided, by cash or Direct Deposit: Account name: Jacqueline Lewis, BSB: 633000, Account Number: 114868607(With name as reference.)
Concession rates available on enquiry.
Extra research/project organization:
If a specific project or extended research is required/requested, this will be charged as face to face contact time.
Reports:
Reports can be written and charged in accordance to NDIS guidelines.
Travel for outreach/homevisits:
20 minutes travel each way will be charged in accordance to NDIS practitioner travel guidelines.
COMMUNICATION
Jacqui may be available for out of session support via email at any stage, and via phone (you may need to leave a message) on Tuesday’s, Wednesday’s and Thursday’s only between 9am and 4pm.
If there is a grievance that you would like looked into and feel you cannot communicate directly with Jacqui, you may email ANZACATA administration@anzacata.org with your concerns.
